Under the direction of the Clinical Nurse Manager or Assistant Nurse Manager and as a member of a medical team, the Nurse Case Manager (RN) will provide phone support and triage, medical case management, serve as a resource for Medical Assistants, LPNs, and Client Services, specialized nursing support for patients and medical providers as well as general and administrative support for other clinical activities such as walk-in triage, assistance with specialized clinics, and other duties as assigned

Representative Duties:

Providing Patient Triage

  • Administers comprehensive evaluation of patient telephone callers and walk-in patients, develops care plan, consults with other disciplines as needed, and implements plan including referrals
  • Advocates for patient care as nurse member of multidisciplinary medical team
  • Care plan is communicated to patients in a timely manner
  • Documents triage encounters in medical record, documenting all appropriate activities and plan

Case Management

  • Delivers personalized services to our patients to improve their care using careful assessment, planning, coordination and delivery of care, and outcome evaluation of the plan
  • Coordinates follow up care for acutely ill patients including primary care and specialty visits, VNA, Infusion companies, and prior authorization for services
  • Coordinates preventative services for defined population including education, vaccinations, monitoring and arranging for lab tests and referrals
  • Evaluates adherence to care plan and intervenes to promote patient’s well-being
  • Documents services in medical record, documenting all appropriate activities and plan 

Providing direct patient care, using nursing procedures as appropriate for a Registered Nurse in an outpatient clinic

  • Includes those both acute and chronic care as appropriate to an ambulatory care setting especially HIV/AIDS (ie opportunistic infections, medication side effects) and behavioral issues (ie medication compliance, safe sex) issues as well as other medical conditions as appropriate for nursing case management
  • Assists with patient care under the direction of appropriate medical provider and/or under nursing scope of practice
  • Provides nursing support for LPN’s and MAs
  • Places and maintains IV lines
  • Administers medications and vaccinations, (PO, IM, SC, PR, ID, INH, IV) per medical provider order
  • Checks supplies for nursing patient encounters and notifies manager or supply ordering designee is additional stock needed
  • Administers all medical tests (e.g. EKG, spirometry, TB-ppd) and procedures as per protocol or medical provider request
  • Works with MA and pharmacy to call in prescription refills or obtain prior authorizations
  • Assists with preventative medicine efforts (e.g.: flu, STD, vital sign checks, occupational health)

Maintains comprehensive knowledge of medical issues

  • Maintains comprehensive knowledge of HIV/AIDS medical issues (i.e. opportunistic infections, medication side effects) and behavioral issues (i.e. medication compliance, safe sex) as well as other medical conditions appropriate to an outpatient population
  • Seeks out continuing education opportunities related to essential job functions
  • Completes continuing education requirements per the Massachusetts Board of Nursing regulations
  • Obtains ACRN or other certifications related to essential job functions within 2 years of hire
  • Understands and maintains universal precautions in all clinical activities

Provides health education

Performs other related duties as required


  • ASN/BSN/MSN and current licensure as Registered Nurse (RN) in the Commonwealth of Massachusetts
  • Familiarity with the LGBTQ community and people living with HIV/AIDS (e.g., ANAC certification) as well as a commitment to community health
  • Experience with electronic medical records strongly preferred
  • Current CPR Certification
  • BLS certification required
  • IV insertion/phlebotomy skills preferred
  • Minimum 2 years experience in an ambulatory care setting preferred
